The Weakened status effect applies to creatures that have been significantly affected by a lasting injury.
Maximum Level: 3
Weakened creatures have disadvantage on constitution checks, including endurance saves.
Furthermore, for each level of Weakened, the creature takes a -1 penalty to its speed and carry weight.
Severe injuries can weaken a creature even after the damage has been healed. A common source of this status effect is significant blood loss caused by bleeding severely or for an extended period of time.
This status effect can't necessarily be countered by medicine or resting. The GM determines possible counters to it depending on its cause. Often, the only counter is time.
